The core problem is among MDs themselves. It's interesting how 'partialist' MDs concerned with 1-2 organs like the kidney, heart or skin somehow ended up at the top of the pyramid compared to the 'generalist' caring for all the organs, the patient as a person and their world.

ESP and femoral blocks in the ER. Maybe I need to up my game (and I have registered for a training) and I POCUS a lot - but in the busy ERs I work these patients mostly aren't going home and there are a ton more waiting to be seen. I'm just not doing them. Honest answer.

If family medicine was 'growing' and well supported, we wouldn't have 2.3M or 16% of the Ontario population without a fam MD!

The other issue is follow-up care. There is no obligation for any specialist in the community after ER care to accept the uninsured patient for follow-up care. ER use, hospitalization and hospital specialist use goes up as a consequence. That is bad for everyone.

Most of these MDs will work in a non 1o care envt. How do I know this? Bc I literally teach them. They’re seeking clinical opportunities where they are paid enough to pay back their $150K debt.
