I have a patient with persistent dysphagia despite a full thickness myotomy and I can see a pseudodivericulum above the diaphragmatic hiatus on egd as well as barium. Don’t do endoflip but that may have provided a clue.

Always thought that CC v4.0's rec to do provocative maneuvers in all patients is going overboard without any evidence.

Now there is evidence:
-Majority of pts, there is no reclassification of pts from v3.0 to v4.0 outside of IEM. 10 swallows is enough!

A great tool for various reasons:
✅Rotates
✅Allows grabbing of vessel prior to burning
✅If there's a large snore at the worst possible time, can also use while closed like a heater probe
