78 yo male with CLL diagnosed two years ago in mediastinal adenopathies (EBUS). Now disease under control except for 4R adenopathy increasing in size.

He also had a renal clear cell carcinoma in 2015.
DX: metastasis.

This is a beautiful example of Barrett's esophagus with high grade dysplasia and complete loss of nuclear staining (null pattern) in keeping with bi-allelic inactivation of the TP53 gene.

WORST mistake is to confuse these metastatic 'Lobular Breast Carcinoma' cells with a lymphoproliferative disorder. (Pleural Fluid, Cytospin)
