Acanthosis nigricans is due to hyperinsulinemia.

Hyperinsulinemia stimulates IGF-1 receptors to induce epidermal keratinocyte and dermal fibroblast proliferation. ⬆️ insulin causes ⬆️ 'free' IGF1 by ⬇️ IGF binding proteins 1 and 2.
