Metastatic endometrial stromal sarcoma to the bladder is extremely rare, can happen many years after the primary resection. The differential diagnosis includes large nested subtype of urothelial carcinoma. History taking could be helpful to order the right IHC #GUpath

70 y/o male with incidentally identified bluish raised lesion in the bladder dome.

#Gupath This is an 82 y/o man. Previously dx with Ca in Situ and cystitis glandularis. They did a follow up cystoscopy and found extensive bluelight positive mucosa that was biopsied. I only see cystitis glandularis here. No significant atypia or mitosis present. Agree ?
