In this study of critically ill adults in ED, the physicians gut impression within 15 minutes performed better than any other existing tool in identifying sepsis

So why do we feel forced to use screening tools which we know cause their own problems?

Is there really conflicting literature on O2 targets in the ICU?

ORs of mortality from all the trials I know of.

There is NO more variation in the outcome than expected based on the imprecision of the effect estimates (τ^2 = 0).

Just consistent evidence against a large ATE

People often talk about the time critical nature of sepsis. But it may surprise you to hear that the evidence to support “early” antibiotics actually isn't a settled debate. Why is that? The key question is “early in relation to what”? 🧵1/n

If there was only one scientific practice I could teach to every scientist regardless of stage or field I think it would be: look at the data. Spot check it. Find a few data points and trace them through to see if they make sense. Look at the raw data. Don't just do analyses.

Generally, we have been unable to replicate these findings under experimental conditions. Moving antibiotic administration into the pre-hospital setting hasn’t been shown to improve outcomes in sepsis, and it would be hard to get them in any faster! thelancet.com/journals/lanre… 16/n

This is why, IMO, research in this area has produced weak results. Isn’t this heresy? Doesn’t every hours delay in antibiotics result in a 7.6% reduction in survival? Doubtful. 13/n

If, against the odds, FSU beats Georgia, they should hang the banner. Contested national championship. Both they and the committee would deserve it.

We had a great Match this year, but a 50% ID program fill rate is a serious societal problem that no one seems serious about fixing.

McCarthy on this throw: 'I was just doing film study all week on No. 25, and whenever he turns his head, he doesn't look at the quarterback, so once I saw him put eyes on Roman, I knew he was only covering the width of his shoulders and I just let it rip.'
