I think we need to establish ethical standards of conduct where we are required to report this type of behavior and incompetence from editors and PIs.

At some point, incompetence is indistinguishable from misconduct.

Note also that serious adverse events (SAE) systematically underreported in literature. 2023 systematic review of esketamine trials found that compared to SAE recorded in ClinicalTrials.gov, 42% were not reported in scientific articles.
