This Easter weekend, REACT have supported 9 care home patients, 6 OPAT patients who required IV antibiotics at home, and 12 'crisis' patients needing vital treatment and care in the community. That's 27 patients who would otherwise be in hospital! #virtualward #admissionavoidance
