The fundamental of this on going debate is whether it is 'safer' for vulnerable patients to be on overcrowded corridor in hospital or lying on the floor in the street waiting for an ambulance. Both are appalling and shows how bad it is. This may not yet be nadir. Shaun Lintern

The real tragedy is that this is no longer news but the accepted 'new normal'. The short term incentives focused on hitting 76% target in March also feel like have led to the predicted April bounce. It is an 'eternal winter' and totally grim.

The right policies could stop this. Increasing 4 hour access standard to 78% from 76% doesn’t work. Instead focus on 12 hour length of stay, it’s already in the contract. Not doing this puts elderly people’s lives at risk
