The diagnosis here is, in my opinion, correct. Continuity of care and 'knowing your patients' are needed. It's the execution of the solution that's the difficult bit; implementation with undertrained, inexperienced staff and looking at crude numbers won't help anybody.

Unfortunately, this kind of thing only confuses patients. I was very seriously acutely ill last year. Both myself and my wife are retired consultants, yet in a world of difficult GP access and huge A&E waits, even with our experience it was hard to know who to turn to first.
