After we saw a beautiful analysis of the Big Data 2 Decide project that included a dataset with affy metrix, outcomes, and details on XRT, we got really excited and contacted our now co-authors to collaborate. Genomics played a role - would GARD? 2/n pubmed.ncbi.nlm.nih.gov/34738049/

It was a pretty heterogenous cohort of HPV+ OPSCC patients so we separated all those that got radiation. Then focused on the one who got what we considered standard dosing (orange dots). As usual - we observe a large spread in GARD relative to dose. 3/n

We then asked how GARD (which is predictive of radiation effect) compared to other prognostic models, and, even alone, it outperformed them. Then - if we ADD the genomic information to our know clinical variables we recover a far superior model! 5/n

Now what if we if we found two optimal dose cut points and randomized patients to a trial of de-escalation? (In the spirit of HN005). Well - we find that if you stratify your randomization by genomic ‘group’ you can achieve equipoise with dose de-escalation! 6/n

We’re pretty excited about this finding - for patients who will receive radiation, GARD has a stronger statistical effect than HPV status did when it first came out - we think calculating GARD should be considered in staging! 7/n

As an aside this is the first cohort done with FFPE, opening the door to many more retrospective cohorts and also making translation easier… 8/8
