Thank you for inviting me. Health Services Safety Investigations Body (HSSIB) see regularly the disproportionate impact on patient safety of inequalities - we all need to come together to tackle this


Today we mark World Health Organization (WHO) #WorldPatientSafetyDay. This year’s theme is improving diagnosis for patient safety, highlighting the critical importance of correct and timely diagnosis in ensuring patient safety and improving health outcomes.


The penultimate session of the #HSJpatientsafety Congress is happening now - Rosie Benneyworth, Melanie Leis Imperial College London & James Titcombe are exploring key trends of the 2024 ‘national state of patient safety’ - what’s changed, what’s getting worse & where can we improve?

Health Services Safety Investigations Body (HSSIB)'s report highlights a litany of concerns over safety, much of it driven by national shortages of mental health staff and warns the flagship NHS long-term workforce plan ambitions may be “unattainable”.

This is an interesting and insightful piece by Health Services Safety Investigations Body (HSSIB) - I think it demonstrates the value of this scanning, aggregation and learning approach. But it’s for the provider and commissioners of services community to pick up the #patientsafety opportunities.
