Having seen many A&E departments up and down the country with Getting It Right First Time (GIRFT) and worked in an overloaded Bolton NHS Foundation Trust last night, I know that we cannot go on as we are. Tinkering around the edges is not enough. Urgent, immediate and radical reform of the whole EC system is required.

We recognise prior experiences and build on the new learning plan in a bespoke process. This has helped to simplify the CESR process. However RCEM and GMC need to come up with easier process. It has proved its worth for 15 years and before its introduction, EDs were unbearable - both for patients and for staff. We must increase staffing and capacity to meet the target - not lower the target to match our inadequate resources!
