Changes to management of hypertension in pregnancy, and attitudes to self-management: an online survey of obstetricians, before and following the first wave of the COVID-19 pandemic doi.org/10.1016/j.preg… #hypertension #pregnancy #BFletcher #KLTucker Richard McManus

"A substantial majority of UK obstetricians considered that self-monitoring had a role in the management of pregnancy hypertension and this increased following the pandemic."

Self-monitoring BP & HTN Dx During Higher-Risk Pregnancy ja.ma/3KCE5Jx Do we think this is true null effect? Or do we need to study a diff population (who wouldn’t otherwise self monitor) @AHauspurg @jennlewey Agnes Koczo Hy Simhan The BUMP Trial JAMA JAMA Network

In this randomized clinical trial, among pregnant individuals at higher risk of preeclampsia, self-monitoring of #bloodpressure with telemonitoring did not lead to earlier clinic-based detection of #hypertension. ja.ma/3OVQxY8 Richard McManus

Delighted that both BUMP trial papers are out today. This is BUMP1 which tested whether self-monitoring of BP plus telemonitoring via an app plus usual care vs usual care alone led to earlier detection of hypertensive pregnancy. 1/7 jamanetwork.com/journals/jama/…

More work is needed to understand if different protocols for measurement and/or antenatal care with self-monitoring might unlock the potential of self-monitoring in pregnancy. 7/7

Delighted that both BUMP trial papers are out today. This is BUMP2 which tested whether self-monitoring of BP plus telemonitoring via an app plus usual care vs usual care alone improved BP control in hypertensive pregnancy 1/6 jamanetwork.com/journals/jama/…

Future work might assess whether different monitoring regimes or organisational interventions combined with self-monitoring might unlock the potential of self-monitoring of BP during hypertensive pregnancy. 6/6

Really delighted that the UDIP study is out today led by Dr Beth Jakubowski and Kath Tucker: Pregnant women visually read dipsticks for urinary protein with similar accuracy to MWs/other HCPs & automated testing was not significantly different doi.org/10.1111/1471-0…

Richard McManus (@richardjmcmanus) 's Twitter Profile Photo

This suggests that recommendations to use automated testing (often unavailable in the community) don't add to visual testing and that given test performance, perhaps longitudinal testing by (higher risk) women might be a sensible way forward (repeating tests improves performance)
