Medicare arose from a failure of early private insurance to cover the elderly. Health spending’s now a fourth of total gov spending and rising. Markets and government failed because they were piecemeal. Playing both sides without a comprehensive goal doesn’t achieve anything.

The services that compete on cost happen to be the ones people don’t expect insurance to cover. Consolidation and price gouging occurs as a strategy in hospitals that negotiate with plans. Seems like a pretty strong correlation to address.
