Having seen many A&E departments up and down the country with Getting It Right First Time (GIRFT) and worked in an overloaded Bolton NHS Foundation Trust last night, I know that we cannot go on as we are. Tinkering around the edges is not enough. Urgent, immediate and radical reform of the whole EC system is required.

A: This patient is a direct referral to AMU, can they come up? B: There aren't any beds, they'll need to go to A&E A: There aren't any beds in A&E either... B: ... A: ... Serious question - why is this ok? Why are EDs expected to absorb this risk?
