Tim Cook Cliff Reid 𝘈𝘯𝘢𝘦𝘴𝘵𝘩𝘦𝘴𝘪𝘢 Good points, Tim. The one point I would disagree with you on is about RCTs not being the answer. With appropriate enrichment strategies, a patient population that would stand to benefit from RSI components (e.g. cricoid pressure) could foreseeably be identified.

Stratified randomisation can help ensure that is the case in smaller trials. Once you go big (e.g. >500), it should balance itself out.

Jan Hansel Cliff Reid 𝘈𝘯𝘢𝘦𝘴𝘵𝘩𝘦𝘴𝘪𝘢 I think tricky …..you’re looking at 10s of 1,000s to get anywhere near useful answer. It would also need to be done in patients genuinely at risk of aspiration (unlike Birenbaum et al) so the issues of ethics and equipoise are significant. …..but you’re the man!
