How is it that sometimes one vessel (here: Cx) seems totally destroyed by CAD while others (RIM, LAD) only have mild disease? Smoking, lipids etc should affect all vessels the same way, right? Anybody understand the underlying pathological process?

Historic milestone in cardiology! On Dec 26, Dr. Raj Makkar at Cedars-Sinai performed the first-ever commercial transcatheter transseptal mitral valve replacement. U.S. FDA had approved of the device earlier this week. Dr. Makkar also led the first research case in 2018.
