Evidence put before the FDA in the year that my son was born. Nothing changed and 29-years later my son attempted to kill himself whilst in an agitated state after his GP put him on Citalopram without warning him.

They were designed for old Twitter where 'shadowbanning' was essentially just replies being hidden behind the 'offensive content' barrier. Now the algorithmic suppression is much more complex. There are degrees of visibility.

Best practice is rarely 'best' practice.

It's just doing the same as everyone else, because this way it feels safer.

And you have to think less.

Entire industries, optimize around quite stupid metrics

Alastair McAlpine, MD Justin Garson Check out this study - most psychiatrists would not take antidepressants themselves, despite recommending it and they are not telling patients honestly what they would do if they were the patient

Could someone provide an example of iatrogenic patient harm that the medical field has proactively addressed?

Male surgeon. Non consensual sterilisation of a woman just after her baby’s birth. “Limited reflection” after the incident from the perpetrator. This is egregious. 1 year suspension.
What is going on GMC?

'I kind of felt like medical school was turning me into a terrible person.' Great podcast with Carl Elliott Carl Elliott about how surprisingly few people are willing to blow the whistle in medicine and the vicious backlash they typically face.

Exact Sciences markets itself as a leading provider of cancer screening and diagnostic tests.

In 2014, the FDA approved the company's product 'Cologuard' as the first and only stool DNA colorectal cancer screening test which people could use at home...

Again, this is not news. In 2017 in The BMJ, Vinay Prasad MD MPH pointed out two then recent studies which showed that between 2008 + 2012 the FDA approved most uses of cancer drugs without evidence of survival or improved quality of life.

My only criticism is: you could have published this a decade ago, maybe more, and it would be identical.

This doesn't make Bloomberg wrong - it makes us bad at reform.

When science is done correctly:
1. Most results should be negative.
2. Most efforts should be spent to falsify promising data (i.e. prove them wrong).
Alas, the system is geared towards showing that everything works, The result? Noise rather than reproducible usable data.

academic medicine is an exceptionally hierarchical, rigid, authoritarian organization

central dogma of academic medicine is moral mission carried out through medical research that outsiders don’t understand

there’s a particular kind of resentment toward people who break ranks

This short 🧵 is just a small example of why “Evidence-based” medicine, as currently employed, is a sham and can’t be trusted. Drug companies run the trials and then massage the data to get outcomes they want, then their goons they’ve bought off at U.S. FDA approve the crap.
