IMPRESSIVE case illustrating transformation of a Papillary Thyroid Carcinoma, Tall Cell Subtype (patient is S/P total thyroidectomy) to an Anaplastic Thyroid Carcinoma 9 years later (FNA of a Level 3 neck node). (Core bx and PAX8, focally positive in more epithelioid areas).

3rd ventricular tumor - 2yo Good morphological evaluation and clinico-radiological correlation is enough to make the diagnosis of Pilocytic astrocytoma (grade 1) But indeed, if 2nd generation BRAF-inhibitors are needed and accessible, demonstrating BRAF fusion becomes mandatory

Previous thyroid case: multifocal PTC, classical and Warthin-like, the left lobe nodule is Langerhans cell histiocytosis, it can be mistaken for solid subtype PTC as both contains nuclear grooves, key point is eosinophils and prominent nucleoli. IHC is CD1a #PathX

Tricky kidney case (for me!) - 1st impression: ccRCC, but ultimately Dx papillary RCC with diffuse clear cell change Clue: many cells are NOT optically clear! Per ExpertPath: "CC change in PRCC is usually focal but may be more diffuse in some cases" CK7+, AMACR+ #GUpath
