Emergency medicine is a lifestyle, not just a job. You’ll see the sickest patients, the worst luck, and the darkest corners of humanity—& still be expected to smile, move fast, and get it right every time. A thread on what they don’t tell you about EM. 🧵

No one trains you for what it really means to be an emergency physician. You will make life-or-death calls in seconds. You will miss things. You will doubt yourself. And you will still have to walk into the next room and be perfect.

The system is broken, and you are the duct tape. When healthcare, law enforcement, mental health services, & social safety nets fail, the patient lands in your ER. At 3 AM. Alone. You will be expected to fix the unfixable.

Your family won’t understand. You’ll come home covered in dried blood & vomit, and have to take out the trash. You'll have to do it with a smile--your family doesn't deserve to carry the weight you bear on your shoulders.

You will see the worst of humanity. Violence. Neglect. Addiction. The child molester with AIDS. The pregnant child with herpes. The maggot-filled bedsores. The psychotic homeless man talking to ghosts. And yet, your job is so smile and soldier on.

Somewhere in all of it, you will find the moments that matter. Holding a dying man’s hand because no one else will. Making a kid with cancer laugh. Saving someone who never thought they’d see tomorrow. These are the moments that keep you going.

Emergency medicine will break you if you let it. The job will never love you back. The hospital will never be loyal to you. Consultants with mock you. Patients will forget your name. But you will remember. You’ll remember who you saved, who you lost, and the difference you

You don’t do it for thanks. You don’t do it for money. You don’t do it for prestige. You do it because, somewhere, deep in your soul, you know you can make a difference. Welcome to EM.
