An easy policy fix is to make the burden of appeal on the payer, not the provider a la some form of “pay first” legislation. Healthcare is unnecessarily expensive because providers have to price in the gamesmanship of payers — automatic down-coding, slow/no pay loopholes, denials

If the burden of appeal was on the payer, the excess dollars in healthcare would find patients, providers, and communities instead of Wall Street, and independent health systems/physicians could survive.
