Postprimary TB typical features of significant Tree in bud combined with consolidation or cavitation in typical upper lobe & superior segment of lower lobe distribution is mostly suggestive. Differentials including fungal or bacterial infection lack significant Tree in bud

Miliary pattern, smear negative active TB. Either primary or postprimary. The latter, more frequent. Associated with childhood & immunocompromised adults. HRCT: randomly distributed nodules reflecting hematogenous spread. Differs from endobronchial spread: centrilobular nodules
