Thanks to everyone who responded. Most of what I was thinking was already said. My take is that if your center has capacity to handle a lot of trauma overtriage, age adjusted activation for GLF is good. When it starts spreading folks too thin, you need an alternative strategy.

This was another component to my thoughts on this matter. Just say yes has always been the mantra of the trauma surgeon & continues to be a motto in EGS & the field of ACS as a whole. At some point, though, we have to acknowledge that we can’t be everywhere at all times.

Rob Winfield “Just say yes” can lead to a job satisfaction challenge. Doing what needs to be done (when others won’t) is morally and ethically rewarding. But you can feel undervalued. Especially when it’s large volume/low acuity.

Potentially unpopular opinion: I don’t disagree that leaders are often chosen for the wrong reasons; that said, it’s hard to lead an academic group with diverse interests if you don’t have at least some experience in the area that one of your followers is interested in.
