Extension of urothelial cancer to prostate: within prostate gland only = T2, with stromal invasion through prostate gland = T4

Reporting lymph node metastasis: number of positive nodes, extranodal extension present/absent, size of largest tumor deposit

Intratubular germ cell neoplasia: clear cytoplasm, nuclear enlargement, prominent nucleolus. Use OCT3/4 to diff btw ITGCN (+) vs other (-)

Seminoma cytologic features: distinct cell border, no nuclear overlapping, uniform cells. #RoTriad

Seminoma gross appearance: homogenous, lobulated, bulging

Seminoma triad: lymphocytes, fibrous bands, clear cells with no nuclear overlapping #RoTriad

Choriocarcinoma: two cell population (mononuclear/multinucleated giant cell), hemorrhage, spatial arrangement (vague villous configuration)

Beware of testis germ cell tumor cells hiding in prominent granulomatous inflammation. OCT3/4 can help highlight them.

Spermatocytic Tumor: Small lymphocyte-like cells, large cells with, soccer ball-like chromatin, intermediate cells

Nots of Spermatocytic Tumor: not assoc with ITGCN, not assoc with undescended testis, not seen outside testis

Helpful features in yolk sac tumor: eosinophilia hyaline globules, basement membrane-like material, Schiller-Duval bodies
