Inquest recommends "holding an independent inquiry, led by a party separate from a health authority, when an unexpected death occurs" in a psychiatric hospital. Much needed! All deaths in psychiatric "care" custody need inquiries--it'd be very revealing. timescolonist.com/local-news/inq…
