Great commentary Simona. Thank you. Minor stroke has historically been difficult to define using NIHSS; non-disabling is a bit more difficult (involves subjectivity). Agree, if not treated, pts can remain at risk of stroke progression and early recurrence.

Agree. I call it transient neurovascular events. We have been arguing best nomenclature for over two decades. 25% of TIAs, have a dwi lesion at follow up. Tmax data can be useful.

This is so true Dr. Lees. How do we encourage current academic leadership that even though research and subspecialization are important, the goal should be to be a "generalist with a specialist eye"? In the US, we are creating "neurohospitalists" as a new specialty!

Great comments, but ability to offer rtPA in delayed time window is a remarkable development. Older DIAS 2, DEDAS, EPITHET failed because of more liberal perfusion thresholds. Distal occlusions are more responsive to rtPA. Many interventionists dont target distal occlusions.
