Sometimes we need to think beyond textbooks & taxonomies. This temporal arteriovenous fistula was so unusual – not dural, not parenchymal, but something in between – that it was misdiagnosed/mistreated for yrs. MCA feeding arteries coalesced to join a varix on middle fossa floor,
