Ever wondered what determines the hemodynamic impact of tricuspid regurgitation? What is the impact of RegVol/RegFr on RA V waves and on stroke volume? What the impact of RA compliance on RA V waves? Can we safely use indirect Fick in tricuspid regurgitation? Circ: Heart Failure 🧵👇

1/ We performed simultaneous RHC and 3D echocardiography in 74 pts: - 3DE --> total RV SV (RVEDV-RVESV), RA volumes (max-min) and RA reservoir strain - RHC --> anterograde RV SV (direct Fick) and RAP waveform - 3DE + RHC --> RegVol and RegFr, as well as RA compliance

2/ We found that: RA V waves were associated with RegVol and RegFr, but this effect was fully mediated by RA compliance!

3/ Respiratory-related changes of atrial pressure matter! TR is associated with inspiratory ↑ of RA pressure systolic amplitude and ↑ RAP/PAWP

4/ Cardiac index is associated with TR severity (RegVol or RegFr) when CI is measured with the direct Fick method. The indirect Fick method overestimated CI proportionally to TR severity! Irrespectively of the equation used to estimate VO2 (Dehmer, Bergstra, Lafarge).
