ā€œAt Baseline.ā€ Came up on rounds today—led to sharing w/ team amazing Paul A. Bergl NEJM article. Just bec pt is back to baseline does not mean baseline is normal. Ask why. Might learn something & be able to help #MedTwitter

In 2019-2020, the *average* US medical student scored a 245 on USMLE Step 2 CK. In 1993-1994, this would have been a 98th percentile score. This is not an April Fool’s Day joke. youtu.be/JS0o8gRahQs

A few ET tubes, a couple central lines, 5 patients, several rapid responses and a code later.. first day of residency (in the ICU) is done. Can’t wait to look back on this day but also savoring how new I feel. #GetLearnt

Note to my future Sodium-fixing self……When treating hyperNa make sure all of the patient’s IV meds are in D5W. When treating hypoNa make sure they are in NS.

#TipsForNewDocs Don't shy away from giving patients bad news- false hope is worse. Offer honest assessment of prognosis + options -> together make a plan. End with a promise: you will still be their doc even if you can't fix their disease. #kittlesonrules

This essay is crazy popular. I am surprised. To me it is pretty simple. 100+ comments in a few hours. drvinayprasad.com/p/when-the-pat…

The management of ascites in patients with cirrhosis can get tricky. When they’re hospitalized, whether the wards or the ICU, making sure we don’t overdo it is a delicate balance. This article details an algorithm for this. šŸŽ© tip to the authors. eddyjoemd.com/foamed/

These days the inpatient service is disheartening. It is sad to see the over medicalization, over treatment, and over testing of our patients. The only thing we don’t over do is actually sitting down and having honest conversations with patients and their family members.
