Mazen Many physicians go to the code status discussion right after the prognosis. First, you need to know the patient as a person. x.com/snakagawa_md/s…

THROMBOCYTOPENIA IN CIRRHOSIS I recently heard a hepatologist proclaim that if a patient does not have thrombocytopenia they do not have cirrhosis. Hmmm. Close but no cigar. 20% of patients with compensated cirrhosis have a normal platelet count.

The SENIOR-RITA trial found that an invasive strategy involving coronary revascularization plus medical therapy did not significantly reduce the risk of cardiovascular death or nonfatal myocardial infarction compared to a conservative medical therapy approach in older patients

Davide Capodanno Our data shows the opposite. Retrospective but much larger cohort size. Using 2,317 adults with a median age of 86 (!) we show how invasive approach is associated with improved survival among older adults with NSTEMI irrespective of frailty status mayoclinicproceedings.org/article/S0025-…

I don’t care what anyone says, an elderly patient with abdominal pain is a medical or surgical emergency until proven otherwise.

DKA with normal glucose with the use of SGLT2 inhibitors is an important thing. Those drugs are gonna be everywhere and they’re never going away and so recognizing a low serum bicarb high and gap with the glucose that’s just a tiny bit above normal is a big deal

ICU/ED Secrets: IKYK but whenever you hear that a patient "tolerates Bipap very well", it's useful to remember that there are 2 ways he may tolerate it: #1: he is comatose/barely responsive #2: he is comfortable/awake or easily arousable & can communicate I see scenario #1
