Stopping RAS-inhibitors had no impact on renal function, blood pressure or safety outcomes. Start deprescribing? No. Stopped drugs were replaced with other anti-hypertensives. No change in blood pressure, no change in clinical outcomes. nejm.org/doi/full/10.10…

How do prediction models perform under data shifts? Great work by Yikuan, Shishir, Reza Khorshidi and Mohammad Mamouei shows that all models loose performance but deep learning model remains on top. What is the solution to the loss? Read here: academic.oup.com/ehjdh/advance-…

Another promising agent for control in resistant . In PRECISION trial, aprocitentan, a dual Endothelin antagonist, provided clinically meaningful lowering of BP over 48 weeks, with excess oedema and fluid retention though. thelancet.com/journals/lance…

So far so good. Baxdrostat, a selective aldosterone synthase inhibitor shows a promising degree of reduction with a good safety profile. This would be a welcome addition to the drugs currently available. Looking forward to the phase 3 RCT. nejm.org/doi/full/10.10…

To call on physicians in to stop treating non-urgent patients, and join the 'people movement' by Saeed Ganjizadeh, is morally wrong, and another act of aggression. If you care about people, call on brutal sanctions to get lifted. youtube.com/watch?v=6HIK1-…. Ali Alizadeh

Agree. Just returned from a trip to Tehran. Amazing how the scattered protests are depicted as the next big revolution in the Western media. This looks like a full blown cyber, media and intelligence war against a sovereign country.

Really exited to share our latest paper - out today in The Lancet with simultaneous presentation:

We wanted to know whether patients with autoimmune disease had a higher risk of developing cardiovascular disease.

A short 🧵

UK clinical research is in more than bad shape

We need a different way of looking at the challenges

We've witnessed the continued decline for >10 years and responded with little innovation or guile to reverse that

The decline continues/accelerates

Perhaps Einstein was right

Our study challenges the adoption of differential thresholds, intensities or drugs in people with and without . This study calls for the removal of specific BP thresholds when selecting people with diabetes for BP-lowering therapy. sciencedirect.com/science/articl…

Impressive! A drug for obesity control that seems to work. Weight ⬇️20%; like gastric bypass surgery.

The 'upstream' treatment shows also remarkable effects on 'downstream' risk factors.
Systolic ⬇️ 6 mmHg (like a classic antihypertensive!).

This could have big clinical implications for patients with Inherited disease

They made >8000 genetic changes that do not change the protein sequence (each amino acid typically has more than one codon)

75% of these do something (not nothing which has been the assumption)
