Patient data store - else all the health related data sitting on your watch won’t be there !

We may have to (politely) disagree then. One patient can be on multiple pathways simultaneously. I am. Others are. But we are still the same unique person. The one constant. I hear your overload, but not everything about them needs to be used by every pathway. Good debate tho'.

Agree - if you start with the person, build out on place-based primary and community (and social) care you reach a different point than starting with a hospital centric model.
