If everyone had Rajesh Rao, MD available, then for sure a PPV and scleral fixated IOL. However, with careful use of OVD and fluidic mgmt, this could be managed via a clear corneal incision approach, with a limited anterior vitrectomy after nucleus sequestration and removal.
