The Universal Care Plan for London is going live London wide on the NHS app & web browser from 23 January 2024. Accessed via your health>care plans. This will enable everyone with a UCP to view it independently via this digital platform . Gary McAllister @lucycolleer Nick Tigere

It’s been a long time coming! From 23 Jan, Londoners with a Universal Care Plan for London will be able to view care plans on the NHS App. This is a huge achievement to integrate with a national platform. Next step is read/write access so patients can share and update their own wishes on the app.

We are pleased to announce that the UCP has integrated with NHS Digital’s National Record Locator (NRL) service. The London Ambulance and the London Urgent Care services can now view UCP care plans via the National Care Records Service (NCRS). #LondonUCP #careplanningsolution

Today, a colleague and I visited Acton Care Centre to see some of the amazing Marie Curie nurses in action. They use the Universal Care Plan for London with all their residents to enable care planning discussions! Every DSPT compliant care home in London can have access! ucp.onelondon.online

Big week next week with release 2.1 incoming. Starting ‘small’ with SCD (~4,000 patients) and then scaling up to dementia, frailty, learning disabilities and autism, BCYP and others later in the year, closely followed by patient editable access via NHS App

June was another record-breaking month for the UCP, with 4,200 new care plans being created and over 19,000 care plans being accessed. There are now over 64,000 patients in London who have a UCP that is visible to everyone who cares for them.
