When performing an epidural, it is useful to think about size of the syringe. As Macintosh shows (Image - 1958), a 2 kg force on a 2 cc syringe will generate 3.5 times more pressure compared to the pressure when using a 10 cc syringe. The educated thumb feels the pressure.

By the way, an oculoplastics specialist does eyeball residency followed by 1-2 years of facial plastics. If an ophthalmologist examines your breasts, please call the police. I’m so glad she is documenting the absolute farce that is peer to peer review.
