I really struggle to understand how this keeps happening. These people know that they are neither qualified nor permitted to be prescribing or requesting ionising radiation. This isn’t a case of “oopsie, my bad”, it’s professionalism issues on an astounding scale.

We are not documenting crowding in individual patient notes. Makes it difficult to give context around delayed care for coroner, solicitor etc Do you document crowding in clinical notes? If so what phrases do you use?
