Dr Paul O'Connor introduces three myths of human factors. #Humanfactors do not equal individual #nontechnicalskills. Systems level changes are also required.

Really engaging Q&A session with Paul O'Connor #humanfactors #IPERN. One size does not fit all. We should not 'blame' the individual, we need to look at the system and we should shift to acknowledge learning from when we do things well, rather than when things go wrong!
