For selective cholangiogrammer: say you're doing a cholecystectomy and NOT doing an IOC (no indication). Do you make a cystic ductotomy and milk retrograde to check for cystic duct stones? Comment on why or why not...

I've coined the term "M+M Vortex": a topic that comes up repeatedly at M+M conference, everyone has a strong opinion, but there's no data supporting any view. Circular arguments ensue indefinitely until moderator moves to next case or hour ends. What is your favorite M+M Vortex??

When air traffic controller tells pilot "cleared for takeoff", they're not making a guarantee nothing will go wrong w the flight. They're just saying 👍 on safety checks to ensure runway is clear, flight path correct, etc. So can we go back to calling it "cardiology clearance"??

Every med student knows that biliary colic is an "indication" for surgery. But these 3 studies show after >150 years, we know little about how to diagnose it + what long-term effects of cholecystectomy are.. pubmed.ncbi.nlm.nih.gov/39167382/ pubmed.ncbi.nlm.nih.gov/37541808/ pubmed.ncbi.nlm.nih.gov/37869171/

This is awesome and IMO something all surgeons need to be doing (structured video review). Problem is reviewing others’ videos is so time intensive. In the future computers can do the grading and provide automated feedback after (during?) cases

For those brave enough to enter the minds of Drs. Christy Dunst + Lee Swanstrom, I dug up some videos from my fellowship of their cruroplasty technique for my talk on "The Difficult Hiatus". During "Getting Out of Trouble During Foregut Surgery" this Thurs 4-5p SAGES is in Tampa in 2026!

Amazing/inspiring to see a resident-led QI project lead to dramatic improvements in pt care (doubling CT sensitivity)!!! NW Gen Surg Residents Northwestern Surgery

Challenge for all cholangiogram-ers out there! Here are 72 IOCs I did in the last year. 🏆1st Prize🏆 goes to whoever can correctly identify most CBD stones. Bonus points for finding any other anatomic/other abnormalities...
