When thinking about C. difficle, we often associate it with the use of clindamycin and fluoroquinolones. However, third and fourth generation cephalosporins are also major risk factors. De-escalating antibiotic therapy is important for stewardship, but also for C. diff #TwitterRx

Doxycycline is often the agent of choice for Lyme disease because it is also effective against Human granulocytic anaplasmosis (HGA), another tick-borne illness that can be transmitted by Ixodes scapularis and Ixodes pacificus (the SAME tick that transmits Lyme) 1/2 🕷️

Consider coinfection of Lyme with HGA if the patient has a higher grade fever for >48hours after starting treatment or has unexplained leukopenia, thrombocytopenia, or anemia. If coinfection is not a concern, other options for Lyme include amoxicillin or cefuroxime 2/2 #TwitterRx

Why are we debating Narcan over the counter, available here or there, etc? Opposition to Narcan is like being opposed to oven mitts; seat belts; rubber things on table corners to keep babies safe. They don’t encourage dangerous behavior. They reduce harm.
