Sometimes I see "triply-avoidable" TACO where ⭐️product wasn't technically needed, ⭐️pre-existing volume load wasn't addressed, & ⭐️product went in too fast, like stacked quality-of-care microaggressions, worse than the sum of its parts. Should we be scoring "preventability"?
