The challenge consists of 3 teams looking at different aspects of the Stroke pathway in #Lincolnshire. 1. Reducing length of stay on the acute stroke unit 2. Seeking seamless transitions of care through different parts of the pathway 3. Living well with Stroke in the community

Stroke is so complex a person has many investigations and treatments while they are in hospital. From pharmacy to the Orthoptic team they are ensuring all team members have their eye on not lengthening a persons stay if they can help it.

Some people some extra help when they go home after a Stroke. Adult social care are going to become even more a part of the team to pre-empt this and ensure the care is ready and waiting so people can go home quickly!

It is easy when working in the NHS and all its various organisations, teams and trusts to forget ordinary people see it as one seamless, integrated, whole. Which is where our Seamless team comes in #lincsstroke100

The #lincsstroke100 Seamless team will begin by gathering service users and their carers experiences and then looking at where duplicated assessments/ processes can be streamlined or stopped. Less form filling and less repetition for patients!

Reminded yesterday how much environment can make to a persons ease of going home after their Stroke. Fortunately there are a lot of great occupational therapists (the experts on home adaptations!) working in the stroke pathway in Lincolnshire.
