In cases with super tortuous arch & low carotid bifurcation, I’ll use a buddy wire (018 Glide Advantage or BMX96) out the ECA in tandem with the working wire during angioplasty & stenting. If stenting the proximal common carotid, try placing the buddy wire out the subclavian.

Tried the new Sofia 88 guide catheter and it tracked really well. At 115cm, it will be particularly great for really tall patients with tough arches Make sure to use a 130cm 5Fr Sim select catheter without a tuohy or with a short tuohy Terumo Neuro
