I’ve been really contemplating going from ICU to L&D. Five years in the ICU and I am burnt out. I’m not happy. Having my son opened my eyes to the L&D world and it would be nice for the majority of my patients to have a positive outcome. Anyone made this switch?

Yesterday I noticed a small amount of vaginal bleeding on my 69 yo F pt, persuaded my MICU team to place a GYN c/s, convinced my pt to go through w/ a transvaginal US, and ~probably~ helped her find advanced endometrial cancer. Women’s health is often ignored. That felt good

Not PT walking a patient around on 0.7 of precedex that is head bobbing/falling asleep to the point where they’re basically dragging them back to bed 😩😭

Caught hyper K+/sine wave on my pts tele and grabbed attending. Said if we did not give bicarb/calcium the patient was going to code. They did not listen. The patient coded. Due to multi system organ failure I’m sure that outcome was inevitable but idk if it had to happen today

Yesterday a fire broke out on my MICU. A high flow, patient, & pt’s bed were on fire. We were able to contain the fire and get 6 pts out of danger in <5 minutes. 3 were intubated, 3 were CRRT, all on oxygen. It was fucking chaos.
