For many very rare diseases, we know our estimates of penetrance can be substantially inflated due to reliance on phenotypically-driven (thus highly biased) case ascertainment. SCCOHT (Small cell carcinoma of the ovary, hypercalcemic type) is a very aggressive (>70% lethality)
