Cutting non-clinical roles may sound superficially attractive but it generates costs through redundancy payments and often does not result in any meaningful long-term financial savings. There is also the risk that the administrative burden on frontline NHS staff will increase.

Safety netting ensures that responsibility for monitoring an evolving illness is shared between clinicians and patients; reducing the risk of a delayed diagnosis or avoidable complications for serious conditions like cancer. Good communication and medical records are essential.

The longer I work as a doctor, the more important I realise that factors such as poverty, education and housing are for people's health status and clinical outcomes. Medical interventions are much more effective for patients for whom these areas of their lives are satisfactory.

Medical care alone cannot overcome the effects of the wider determinants of health like poor living conditions, limited income, insecure housing or low health literacy. All these factors shape both the development of illness and a patient’s ability to follow treatment plans.

The same intervention can be highly effective for someone with stable life circumstances and yet much less so for someone facing social disadvantage. Health outcomes depend as much – if not more - on the conditions in which people live as on the treatments doctors provide.
