Living up in the hills in #Hawaii is weird. It was just sunny in my front yard with a downpour in the back. Then vice versa.

Seeing a patient in the ICU, struggling to breathe. Me: "Will you get vaccinated after you get better?" Him: "No. I'm more worried about the vaccine than #Covid19."

The Click Trial (chlorthalidone in CKD4) is very helpful but what to do if someone is already on an SGLT2i? Unclear from paper. Risk of volume depletion? Rajiv Agarwal MD, MS

Avoid cana but consider empa/dapa, look at baseline risk. If heavy proteinuria would discuss costs/benefits with patient for shared decision making. If low grade would most likely avoid.
