I’m receiving a lot of physicals for spring sports with missing pages. Please remember when you are making a copy of something that has front and back pages you have to hit 2-2 sided when you copy or I’m going to be missing half of the physical forms. Thank you.

Have to brag on my school. Something came up so I had Liam in the dugout at baseball game for about 10 mins and the coaches made sure the boys on the fence had a glove to protect him from wild foul balls 🥰 they also gave him a pack of gummies. Luckiest working mom in the world❤️

💥FREE PHYSICALS💥 May 5th 8:30-11:30am at BCHS. Physical packets can be picked up in AT room or front office. This will cover the athlete for the 23/24 school year. Parent/guardian must fill out the forms and send back with athlete on the 5th to be eligible.

Don’t forget to grab your physical packet outside of the AT room located in the gym by Coach A’s office. FREE PHYSICALS MAY 5th 8:30-11:30 You cannot get a physical without this packet filled out by parent/guardian

This is the QR code that will take parents/guardians to the required online forms for 23/24 sport participation forms. Paper physical forms are still required and need to be filled out then pictures can be uploaded here The school will no longer be keeping hard copies of physical

If your student athlete got a free physical at BCHS this morning THE PHYSICAL WAS SENT HOME WITH THEM. You must scan the QR code that I attached to their physical to finish filling out the required athletic participation forms and upload a picture of your physical.

If your child got their physical 5/5 with KDMC they were sent home with a copy with a QR code. Athletic participation forms are now on Google and must be filled out online and pictures of physical must be uploaded. Athletes can no longer turn in physical to coach/athletic trainer

I need to apologize to parents/guardians KHSAA emailed 5/16 & changed not only the physical form but how medical professionals at the school can collect and store them. With that being said we are no longer allowed to upload physicals online. I will be collecting them.
