If systemic hypotension was the sole cause of ischemic hepatitis, every patient in the MICU would have ALT >500 But the incidence is 1 in 50 You need right heart failure, congestion, that exceeds portal pressure. Then the systemic bp drop can be transient or relative

The inpatient diuretic plan: Fumble between high dose IV lasix causing azotemia, stopping to let the sCr fall, then start again when patient is hypoxic/gaining weight. Then stop again briefly. And in a fit of max frustration discharge next day on a low dose of oral nothingburger.

It must be that time of year again when every 75 y/o patient with CKD attributable to: DM, obesity, HTN, met. syndrome has a SPEP randomly checked and there is an IgG kappa MGUS with low M-conc. that you now have to worry about forever.
