A JPSM SR concluded that ketamine was effective for cancer pain. We point out in a letter that most (32/35) RCTs were at high risk of bias (often more than one), were small (<50), and only 5 of 18 trials in efficacy analysis showed any benefit. Most SR of pain are misleading.
